Do You Have A
Health Care Proxy?
A “Health Care Proxy” provides a way to make the very
difficult issues of end-of-life care easier by allowing the
expressed wishes of the patient to be followed in a proper manner.
It can help remove much of the stress that these decisions bring
to loved ones and family members. As practicing Catholics, having
a Health Care Proxy, helps our passing become what it should be -
a hopeful, glorious journey to life everlasting in the company of
our Lord and Savior.

St Patrick’s of Smithtown
Religious Formation
Confirmation Program
St. Patrick’s Confirmation Program
St. Patrick’s Religious Formation Center works with two
distinct groups of candidates. Students of St. Patrick
School, and students of public and other private schools who
attend religious formation classes at St. Patrick’s parish.
St. Patrick School students receive their instruction in the regular
classroom setting, while others attend regularly scheduled classes
sponsored directly by the Religious Formation Center (RFC).
St. Patrick School teachers and the RFC catechists make use of the
same confirmation books and follow the same curriculum (as closely as
possible) during the preparation of candidates for Confirmation.
Our hope is to keep all students as unified as possible in this
preparation process. There are differences in the two tracks
of instruction, especially in the areas of class schedules and
special events. More information can be found below.
Please make sure to select the appropriate track
and level for the Candidate.
Select “St. Patrick School Candidate” if the
Candidate attends St. Patrick School.
Otherwise select “Religious Formation Center(RFC)
Candidate” if the Candidate
does NOT attend St. Patrick School.
Grade/Level 7 Students
(1st Year Confirmation Candidates)
Each October, the St. Patrick’s Religious Formation Center and
St. Patrick School are pleased to welcome a new group of students
to the 2 year preparation program for the Sacrament of Confirmation
- an opportunity to deepen the Candidates commitment to Jesus and
His Church. In Confirmation, the Holy Spirit brings us closer
to Christ, brings us into closer relationship with the Church, and
helps us witness to our faith in what we say and what we do.
Grade/Level 8 Students
(2nd Year Confirmation Candidates)
Each October, the St. Patrick’s Religious Formation Center and
St. Patrick School welcome back last year’s Level 7 students to
continue their 2 year preparation program for the Sacrament of
Confirmation - an opportunity to deepen the Candidates commitment to
Jesus and His Church. In Level 8, the Candidates will continue
their Faith Journey towards the Sacrament of Confirmation that they
will be receiving in the Spring. The work that the Candidates
have done along with their families, sponsors, teachers, catachists
and friends continues to move forward the process of being a
Catholic Christian and of assuming a well deserved place in the
adult Catholic community.
Sponsors of Confirmation Candidates
Welcome Sponsors! You have been recognized by your candidate
and his/her family as a person who has faith and is commited to
Jesus, His teachings and His Church. A Confirmation Sponsor
is a person who lives the Gospel message of Christ. A person
who is a baptized, confirmed Catholic, a registered member of a
parish and an active regular participant of Sunday Eucharist.
A sponsor is responsible for helping to build relationships with
young people. These relationships allow young people to
know adults who care about them and who are willing to walk as
fellow pilgrims on a life-long journey of faith. A sponsor
must be very much a part of a Confirmation Candidate’s
life. They must establish a regular time to be with the
young person. A sponsor is willing to participate with the
candidate in community service activities and also explore with the
young person a variety of ways to pray without feeling uneasy.
A sponsor should share how their faith and experiences of Church
have made a difference in their life.
To sum up, the Confirmation Sponsor will be assisting the young
person in being a true witness and disciple of Jesus and
representing the community when they present their candidate to
the Bishop. The special role of a sponsor should not end at
Confirmation - a sponsor should continue to encourage the young
person to fulfill his/her baptismal promises after Confirmation
with the help of the Holy Spirit.
Note: According to the Code of Canon Law specifies that
the person (sponsor) must have completed their sixteenth year, be
fully initiated, live a life of faith, not be the parent, and not
be bound by any other canonical restrictions.
The Sacrament of Confirmation...
-
welcomes us and deepens our belonging to the faith community
-
deepens our relationship to God, as Creator, Redeemer and Spirit
-
strengthens us as disciples
-
A sacrament is part of the way God loves us and calls us.
A sacrament is God’s love expressed as free, undeserved gifts.
-
The Gift within Confirmation is that of strengthening us for
discipleship and deepening our belonging to the faith community
through the Trinity.
-
God uses visible signs to communicate invisible love.
To communicate God’s love, God uses our senses: eyes, ears,
touch, smell, and tase, and elements of our world.
-
In Confirmation, the signs we will celbrate include being
anointed with oil and the laying on of hands as we remember the
water of Baptism and nourishment of the Eucharist.
-
God calls and invites; we respond. An attitude of
openness communicates a “yes” to God’s invitation
to a deeper relationship. This “yes” does not
mean we know that we will always be faithful or that we are worthy
of the gift. “Yes” means we are open to be strengthened
by God who calls and loves us.
-
In Confirmation,we have the opportunity to say “yes”
through the various responses in the prayer.
-
The community witnesses to and celebrates the sacraments.
A sacrament is a powerful sign that God is alive and present in the
world. As Church, we are called to be sacrament, that is to be
God’s hand and healing touch in the world. At the same
time, we celebrate the sacraments as our most important prayer.
This community that celebrates sacraments is global, reaching to
other faith communities throughout the world and joined in a common
bond. The community is also local as a diocese or archdiocese,
as a parish and as a community of faith in families.
-
In Confirmation, the community witnesses our celebration and
provides a sponsor who represents the love of the community
for us.
